3. Signed Performance Rating Sheet in the last rating period (July-December 2023);
4. Authenticated eligibility such as: a.) Authenticated CSC certificate of eligibility; b.) Authenticated PRC Board Rating and updated License
5. Authenticated Transcript of Records for Bachelor's degree and Postgraduate degree;
6. Certified photocopies of Training certificates;
7. Certified Photocopy/ies of Special Orders for designations.
8. DENR employees who have previously uploaded authenticated documents are not required to do so again. However, entry-level applicants and those disqualified for non-authenticated documents must still upload the necessary documents.
